The term “D-SNP” is short for Dual Eligible Special Needs Plan. Our plan is called PacificSource Dual Care (HMO D-SNP). It is a Medicare Advantage plan with extra benefits at no additional cost to people who qualify for Medicare and full Medicaid. It combines the benefits of Original Medicare, Part D prescription drug coverage, and Medicaid benefits.
No-cost benefits and extras include:

- $0 select drugs
- Medicaid-covered dental care with many routine procedures
- $200 per year for eyewear and $0 routine eye exam

You can enroll in PacificSource Dual Care (HMO D-SNP) if:
- You qualify for Medicare Parts A and B
- You’re eligible for or enrolled in full Medicaid benefits
- You live in our service area: Clackamas, Crook, Deschutes, Hood River, Jefferson, Lane, Marion, Multnomah, Polk, Wasco, or Washington Counties, as well as North Klamath ZIP codes 97731, 97733, 97737, and 97739
